Most insurance plans include physical therapy benefits. Your exact benefits will depend on your specific plan, and you may be responsible for some of the charges such as a copay or deductible. We will help you learn about your benefits but we also recommend that you call the customer service number listed on your insurance card and ask the following questions:
Do I have physical therapy insurance benefits?
What is my deductible and has it been met?
How many sessions per year does my health insurance cover?
Is there a maximum dollar amount covered per year?
What is my responsibility (copay, coinsurance) per physical therapy session?
Is a prescription or referral required from my primary care physician?
Is authorization required from my health insurance company?
Our providers are contracted to provide services with most major insurance companies, including Regence Blue Shield, Premera Blue Cross, KPS, Medicare, Aetna, Cigna, First Choice, Uniform Medical Plan, GEHA, L&I and OWCP.
Charges will vary from session to session as the charges are based on what procedures are performed within your appointment and how long you are here. For those without medical insurance, we offer a cash discount.
Cash, check and all major credit cards are accepted for payment. Payments should be mailed to PO Box 4733, South Colby, WA 98384. You may make an on-line payment by clicking on the Make a Payment tab on this website. We are happy to set up a payment plan for you if requested. Just give us a call.
Missed Appointment Fee
If you do not attend your scheduled appointment and you have not provided at least 24 hours of notice, you may be charged a fee. Please refer to our cancellation policy for more information.
